Procedure for Filling Out an Industrial Accident Report According to Form H-1

Form H-1 is filled out with text and digital information, which must be recorded and coded in accordance with generally accepted (established) terms and a specially developed classifier. Coding is carried out by the organization where the accident occurred.

Point 1

• The first line indicates the date and time of the accident. The day is coded with two digits, the month by its ordinal number in the year, and the year by the last two digits.
• In the third line of the point, it should be indicated and coded how many full hours from the start of work (shift) the accident occurred with the victim.

Point 2

• The first line indicates the name of the organization where the accident occurred.
• The name of the shop (section) of the organization where the accident occurred must be provided in accordance with the approved list of structural subdivisions of the organization.

Point 3

• This point is filled out with text information and is not coded.

Point 4

• The name and address of the organization that sent the employee are indicated.

Point 5

• The first line fully records the surname, first name, and patronymic of the victim. Gender is coded: 1 — male; 2 — female.
• In the third line, the age is indicated and coded (the number of full years the victim had reached at the time of the accident).
• In the fourth line, the profession (position) is coded according to OKPDTR. If the victim has several professions, the one during which the accident occurred is indicated.
• In the fifth line, the length of service is indicated and coded (the number of full years of work during which the accident occurred). If the length of service is less than a year, the number of months and days worked is indicated in the text part, and it is coded as 00.

Point 6

• This point is filled out in accordance with GOST 12.0.004-90, "SSBT. Organization of occupational safety training. General provisions." It is not coded.

Point 7

When describing the circumstances of the accident, a brief description of the working conditions and the actions of the victim should be given:
* State the sequence of events preceding the accident.
* Describe how the labor process proceeded.
* Indicate who supervised the work.
* In the second line, the type of incident is indicated and coded.
* In the third line, the causes of the accident are indicated and coded.
* In the fourth line, the full name of the equipment is provided.
* In the fifth line, the victim's state of alcohol intoxication is indicated.

Point 8

• The persons who committed violations of regulatory requirements for occupational safety, as a result of which the accident occurred, are indicated.

Point 9

• This point is filled out with text information and is not coded.

Point 10

• Each measure to eliminate the causes of the accident is indicated. Penalties imposed on persons who committed violations of state regulatory requirements for occupational safety should not be included in this point.
